Register Your Kit

If you are registering for more than one screen, you need to submit a form for each individual kit.

Your healthcare provider must complete the requisition form prior to screening.

  • If Yes, please select what color of the EvolveGene collection kit that you have.
  • Your unique barcode can be found on either the side of the collection kit or specimen tube, or on your requisition form or registration card.
